患者さんの異変から見えた、意外な原因

先日、歯茎の腫れを訴える患者さんが来院されました。その方は、歯磨きも丁寧に行い、特に大きな虫歯治療の経験もないとのことでした。口腔内を拝見すると、確かに歯茎が腫れていました。詳細を伺うと、最近高血圧の薬を飲み始めたとのこと。そこから、薬物による歯茎の腫れという可能性が見えてきたのです。

薬物によって歯茎が腫れる？ 知っておくべき薬の種類

薬物によって歯茎が腫れることがある、ということをご存知でしょうか？ 特に、以下の種類の薬を服用している方は注意が必要です。

• Ca拮抗薬 (ニフェジピン、アムロジピンなど)
• 抗てんかん薬 (フェニトインなど)
• 免疫抑制剤 (シクロスポリンなど)

聞き慣れない薬もあるかもしれませんが、意外にも多くの方が服用している薬です。

なぜ薬で歯茎が腫れるのか？

これらの薬は、歯茎の細胞に影響を与え、歯茎の腫れを引き起こすことがあります。ただし、薬を服用しているからといって、必ず歯茎が腫れるわけではありません。

大切なのは、丁寧な歯磨きと早期発見

薬物によって歯茎が腫れている場合でも、まずは丁寧な歯磨きを心がけることが大切です。歯垢（プラーク）を取り除くことで、歯茎の炎症を抑えることができます。ただし、焦って強く磨きすぎると、歯茎を傷つけてしまう可能性があるので注意しましょう。

また、歯茎の腫れが気になる場合は、早めに歯科医院を受診し、相談するようにしてください。

Today, I’d like to share a case of a patient whose gums became swollen due to medication.

When we think of gum swelling, it’s usually caused by inadequate brushing and plaque buildup. However, there are exceptions.

This particular patient came to our clinic because they were concerned about swollen gums. Since the patient was good at brushing and didn’t recall having any major dental treatments, I found it odd when I received the call. But upon examining their mouth, I confirmed that the gums were indeed swollen.

The patient reported brushing daily and even flossing regularly. This made the situation even more puzzling, so I asked if anything had recently changed in their life. That’s when they mentioned that they had started taking medication for high blood pressure.

Gum swelling caused by medication
Yes, gum swelling can be caused by certain drugs. Some examples include:

• Calcium channel blockers (e.g., nifedipine, amlodipine)

• Antiepileptic drugs (e.g., phenytoin)

• Immunosuppressants (e.g., cyclosporine)

These may sound unfamiliar, but they’re more commonly prescribed than you might think.

If you maintain proper brushing habits, even drug-induced gum swelling typically doesn’t lead to serious problems. On the other hand, if you panic and brush too aggressively, you might damage your gums and cause further inflammation.
