「先生、この痛み、もう何年も付き合ってるんです…」

「色々な歯医者さんに行ったけど、良くならなくて…」

日々の診療で、患者さんの切実な声に触れるたび、私は【診断】というものの重みを改めて感じます。

一見、原因が分からないような症状の裏には、【見過ごされた真実】が隠されていることが少なくありません。

今回は、スウェーデンのイエテボリ大学の診断学教室に留学し、歯周病と根管治療の専門医を取得後、30年近く臨床に携わってきた私だからこそ語れる、【診断】の奥深さと、その【真価】についてお話ししたいと思います。

▼ 診断は、【闇夜を照らす灯台】

「診断」とは、患者さんの症状、検査結果、そして生活背景など、あらゆる情報を総合的に判断し、病気の【本質】を見抜くことです。

それはまるで、

• 【暗闇の中】で進む船を【安全な航路】へ導く【灯台】
• 【複雑に絡み合った糸】を丁寧に解きほぐし、【真実】を明らかにする【探偵】

正確な診断があってこそ、

• 【無駄な治療】を避け、患者さんの心身の負担を軽減できる
• 【隠れた病気】を早期に発見し、重症化を防ぐことができる
• 【患者さんのQOL（生活の質）】を向上させることができる

のです。

▼ 診断を誤った【悔恨の念】と【学び】

実は、私もまだ留学前に何度と、診断を誤ってしまった経験があります。

それは、私がまだ経験の浅い歯科医だった頃。

奥歯がズキズキ痛むという患者さんに対して、レントゲン写真だけを見て、「虫歯が神経まで達している」と判断し、すぐに神経を取る治療を始めました。

しかし、治療を続けても、患者さんの痛みは一向に改善しません。

そこで、私は、改めて患者さんの【生活習慣】や【過去の病歴】などを詳しく聞き取り、口腔内を【丁寧に触診】しました。

すると、患者さんが、無意識のうちに【歯を強く噛み締める癖】があることが判明したのです。噛み締め癖によって、顎の筋肉に【過剰な負担】がかかり、それが痛みの原因だったのです。

この経験から私は、**【レントゲン写真や検査データだけでなく、患者さん自身を診ることの大切さ】**を学びました。

▼ スウェーデンで培った【診断力】が【人生を変える】

スウェーデンのイエテボリ大学の診断学教室にて、【診断学】という学問を深く学びました。

そこでは、

• 【疾患】を徹底的に理解すること
• 【多様な検査方法】を習得すること
• 【患者さんの訴えに真摯に耳を傾ける】こと

の重要性を叩き込まれました。

スウェーデンで培った【診断力】は、帰国後の私の歯科医人生を大きく変えました。

例えば、ある患者さんは、２０年【原因不明の歯痛】に悩まされていました。様々なクリニックを受診されましたが、原因が特定できず、諦めていました。

しかし、当院で診察したところ、**【過去の被せ物治療の不備と歯茎の炎症】**が原因であることが判明しました。

以前の治療で、前歯のブリッジが歯茎の中深く作成されており、しかも歯にぴったり合っていなくて【感染源・プラーク】が残ってしまい、それが慢性的な炎症を引き起こしていたのです。

そこで、私は、マイクロスコープを用いて、【プラークコントロール】を行い、感染源を徹底的に除去しました。

治療後、患者さんは、長年の痛みが嘘のように消え、【まるで別の人生を歩んでいるようだ】と、大変喜んでくださいました。

この経験を通して、私は、【診断力】が、患者さんの【人生そのもの】を変える力を持っていると確信しました。

今回は、ベテラン歯科医の視点から、【診断】の重要性について解説しました。

診断とは、単なる病名当てゲームではありません。

それは、患者さんの【過去】、【現在】、そして【未来】を繋ぐ、羅針盤なのです。

私がスウェーデンで学んだ【診断力】は、患者さんの【人生そのもの】を変える力を持っていると信じています。

確かな診断力があれば、

• 【隠れた病気】を早期に発見し、重症化を防ぐことができる

Don’t Overlook It! From a Single Cavity to Systemic Disease? A Veteran Dentist Reveals the Profound Importance of “Diagnosis”

“Doctor, I’ve been having a lot of shoulder pain lately…” “I’m worried because these mouth sores just won’t heal…”

In my daily practice, I hear a variety of concerns from patients.

While seemingly unrelated to oral health, these complaints can sometimes point us in surprising and important directions when guided by the compass we call “Diagnosis.”

In this post, I want to share the profound importance of [Diagnosis], drawing from my experience of over 30 years as a dentist, treating countless patients, and providing specific examples.

(I have been practicing dentistry for over 30 years, and have treated many patients. From these experiences, I have keenly felt the importance of diagnosis and make an effort to constantly acquire the latest knowledge and skills. I am also actively involved in academic activities such as the Japanese Dental Association, the Japanese Society for Orofacial Pain, and the Japanese Society of Endodontology, to contribute to the development of dental medicine.)

▼ Diagnosis is Like a [Map of a Labyrinth]

“Diagnosis” refers to identifying the [Cause] and [Condition] of a disease based on a patient’s symptoms and test results.

To put it another way, it’s like:

• A [Map] that shows the whole picture of a [Complex Labyrinth]
• A [Compass] for a ship sailing through [The Fog]

Without accurate diagnosis, it can lead to irreversible events, such as:

• [Misguided Treatment] that worsens the symptoms
• [Serious Illness] that is overlooked and becomes untreatable

▼ A [Bitter Experience] of a Mistaken Diagnosis

Before studying abroad, I once made a diagnostic error. In the case of a patient who came to the clinic complaining of throbbing pain in the back teeth of the lower jaw, I looked at the X-ray and decided that the tooth decay reached the nerve, so I started treatment to remove the nerve immediately.

However, even as treatment went on, the patient’s pain would not go away.

Then, after thoroughly examining the area around the tooth, I noticed that the gums were severely inflamed.

From this experience, I keenly felt the importance of [Grasping the Entire Image of the Patient’s Oral Cavity, and the Whole Picture of the Patient], not just the symptoms of the tooth.

▼ Diagnostic Skills [Change the Patient’s Life]

In recent years, dental technology has made remarkable progress, and various treatments have been developed.

However, no matter how excellent the treatment method is, it can only be effective with [Accurate Diagnosis].

For example, one patient had been suffering from [Unexplained Toothache] for many years. She was even taken to a major hospital in an ambulance.

Despite visiting various hospitals, the cause could not be identified, and she was about to give up.

However, after examining the patient at our clinic, we found that [A Clenching Habit] was the cause of her toothache. As a result of training to open her mouth properly, the patient’s pain improved dramatically, and she even said that [It Had Changed Her Life].

Diagnosis is not simply a game of guessing the name of a disease.

It’s a compass that connects a patient’s [Past], [Present], and [Future].
